Confidentiality:  Any personal information provided to our organization is not shared outside of Guardian Angels for Soldier's Pet and Affiliated Chapters and is considered as confidential information and only shared with those within the organization that has a need to know.  We follow OPSEC (Operations Security) guidelines in regards to confidentiality and sharing of information. Your name and telephone number is only shared with a potential foster home caregiver upon your permission to do so.

Please provide the following requested information about your beloved cat. 

One submission required per cat needing a caring, loving, and safe environment while deployed or emergency hardship.  In the "Other Pertinent" information section at end of the form, please let us know if your pets can be placed with different foster home caregivers or desire them to remain together.  All attempts will be made to keep your pets with the same foster home caregiver when possible.

Submission of this document is not a guarantee that we can place your beloved pet or pets.

Pet or pets must be spayed/neutered prior to acceptance by Guardian Angels For Soldier's Pet.  This is for protection of your pet, current pet(s) in the foster home, and pets of others within the same area as the foster home caregiver is located.
